Purpose: Hydrocephalus, abnormal accumulation of CSF in the ventricles, is one most common diseases brain. Ventriculoperitoneal shunting commonest procedures for management hydrocephalus. Assessment and evaluation clinical outcome, following VP shunt, should be carried out to decrease rates complications this procedure. Clinical audit plays a vital role identifying developing effective methods improving patient care. This paper sheds light on 37 neurosurgical patients with hydrocephalus who underwent ventriculoperitoneal shunt surgery. Our aim was identify defects steps by from time admission discharge.
 Methodology: In study were selected between November, 2021 December, 2022 at West Erbil emergency hospital teaching hospital. surgery performed them.
